Santa's Visit Application

Against the current organisational "wisdom" Santa has now been nationalised. This means that anyone who wants presents for Christmas will have to fill in a form.
For your convenience I attach the form herewith.

I do not claim that it is an Australian decision to nationalise Father Christmas, but I do think that it is a cleverly designed form. The above is actually 3 gif files joined together and does not print clearly.
